The UCLA Police Department Records Unit provides Live Scan/background check fingerprinting services for campus affiliates as well as the general public. The requesting agency/department needs to provide you with the appropriate paperwork for your background check. We are not responsible to determine the type of background check or paperwork to be processed. Please come with all forms already filled out. If you are missing paperwork or over 10 minutes late to your appointment, you will be rescheduled. Your appointment will take approximately 15 minutes. You must bring: 1. A valid government issued ID (Driver's License, State ID or Passport) 2. Request For Live Scan Form from the requesting agency or Ink Cards (ink rolled prints only) We only provide FD-258 ink cards *We currently do not provide cards with the Privacy Act Statement listed on the back of the FD-258 cards, which can result in a submission rejection. Please check with your department/agency before using our cards. 3. UCLA Departmental Recharge Form (UCLA employment only) or form of payment (Credit Card or Check made payable to "UC Regents") FEES: The cost for Live Scan services includes a rolling fee (which varies per Live Scan location), plus applicable State and Federal charges. The rates are currently as follows: •Rolling fee: $25 •DOJ (Department of Justice) charge: $32 •FBI (Federal Bureau of Investigation) charge: $17 *Please note: additional fees may be added depending on the Live Scan form. All rates are subject to change depending on the type of application being processed. Once you arrive and the information provided on your form has been entered into the live scan system, you will be advised of the exact cost. Ink rolled prints is $25 for two cards and $10 for each additional card. Parking: The UCLA Police Department is located on the northwest corner of Charles E. Young Drive South and Westwood Plaza, just north of the Ronald Reagan UCLA Medical Center. Live Scan clients may park in the 15-minute parking spaces located in the front of the police station. If no parking is available, clients may purchase temporary parking from the parking kiosk on Westwood Plaza just north of the police station. Short-term paid parking is also available at the top of Parking Structure 8.
